Most of the time when you work shifts on Clipboard Health, you will submit an in-app digital timesheet to get paid accurately and instantly. Importantly, digital timesheets allow you to make basic adjustments to your clock in/out or break times while still allowing you to get instantly paid (during the facility signature flow after clocking out) [1].
Sometimes, you may encounter a problem during your shift that makes it impossible to get paid accurately using an in-app digital timesheet. For example, your phone may run out of battery at the wrong time, you may be asked to continue working after you clock out in-app, or you may face other technical issues. In these rare cases, you'll need to fill out a paper timesheet to get paid accurately.
Follow the steps below to learn how to submit a paper timesheet. Please note any additional pay from a paper timesheet may take up to two business days to be approved. Delays are typically the result of submission mistakes which can be avoided by carefully following the steps below.
How to submit a paper timesheet
Step 1: Determine the correct paper timesheet form
- If you worked a shift in California, you must use a CDPH-530 form
- For all other shifts, you can use this Clipboard Health paper timesheet form. Alternatively, if the facility has a generic paper timesheet or "missed punch" form readily available, you can use that as long as you fill out all of the required info, per below.
Step 2: choose how to get your paper timesheet signed
There are two ways to get your paper timesheet signed:
- In-person at the facility with a physical paper timesheet [recommended]
- Via in-app facility chat using a digital copy of your paper timesheet
We recommend getting a signature for your paper timesheet in-person if possible so you don't encounter delays, but if you already travelled home and do not wish to return to the facility, you can communicate with the facility over in-app facility chat to get your paper timesheet signed.
How to get your paper timesheet signed in-person at the facility
Please work with a facility staff member to fill out and sign a physical copy of your paper timesheet. See Step 3 below for required fields.
How to get a digital copy of your paper timesheet signed via in-app chat
- Complete all required fields on your paper timesheet EXCEPT the name of the authorized signee and their signature. You can do this by either a.) printing out a physical paper timesheet at home, filling it out with pen, and taking a photo of it; or b.) digitally editing a PDF copy of your paper timesheet (e.g., using Adobe Acrobat, Google Drive, Microsoft Word, or the Apple Preview app). See Step 3 below for required fields. The digital copy of your paper timesheet must be a JPEG, PDF, or PNG file.
- Share the digital copy of your paper timesheet with the facility via in-app facility chat and ask for a signature. You should remind the facility of why you are submitting a paper timesheet (include relevant details and names of staff members).
- The facility must sign and re-upload your paper timesheet via facility chat. They can do this by either a.) printing out your paper timesheet, signing it in ink, taking a photo of it, and uploading to facility chat; or b.) downloading the digital paper timesheet, signing it digitally, and uploading the signed digital copy to facility chat. If the facility is taking more than 2 business days to get back to you, please politely follow up with them. If you are not able to get in touch with them after messaging them, please contact Clipboard Health support via in-app chat.
- Once you receive the countersigned copy of your paper timesheet, you're almost there! Now you'll just need to submit your paper timesheet. Continue to Step 4 ("Submit your paper timesheet") below.
Step 3: fill out your paper timesheet
Please make sure all of the below fields are complete and accurate. Any incorrect information may result in payment delays

Date of the shift (use the start date of the shift if it was a NOC shift)

The full name of the facility

First name of authorized signee (e.g. Supervisor, DON, scheduler)

Your signature and the signature of the authorized signee

Step 4: Submit your paper timesheet
There are two ways to submit your paper timesheet to Clipboard Health support for verification: via "Unverified Shifts" or in-app support chat.
Submitting your paper timesheet via Unverified shifts
This option will only be available if your shift was "unverified", meaning we did not attempt to pay you at all for your shift.
- Select My Shifts (bottom menu)
- Select Unverified Shifts at the top
- If you do not see the relevant shift in Unverified Shifts, or do not see the 'Unverified shifts' section in My Shifts at all, please proceed to "Upload your paper timesheet through in-app support chat" below
- Tap Upload your timesheet
- Attach a photo of your paper timesheet and select Upload
- Note: if you used in-app chat to get a digital copy of your paper timesheet signed, you may submit a screenshot of your paper timesheet here.
Submitting your paper timesheet via in-app support chat
If you do not see the option to upload a photo of your paper timesheet via Unverified Shifts per above, you should submit your paper timesheet via in-app support. Simply start a chat and submit your completed paper timesheet as an attachment. You should be directed to the payments team and receive confirmation that they've received your request.
Step 5: Get paid!
After you've submitted your paper timesheet, it may take up to two business days to approve your request. Please ensure you stay up-to-date with any threads with Clipboard Health support in case there are problems processing your paper timesheet.

[1] A few facilities on Clipboard do not permit in-app timesheet editing. If you work a shift at one of these facilities, you'll need to submit a paper timesheet if you need to make any adjustments to your timesheet.
