Before you can start booking shifts through Clipboard Health, you will need to verify or upload several core documents and certifications through the Clipboard Health app.
- Your professional license # or certificate
- BLS / CPR certification
- Your driver's license or state ID
- TB Test (taken within 12 months) or Chest X-Ray (taken within 5 years)
- OIG & SAM
- Background check (varies by state)
Note:
- You will need to upload a background check via the Clipboard app to ensure the safety of patients and staff. We use Checkr, a secure and compliant service, to perform these checks at county, state, and federal levels. Clipboard covers the cost, and an annual background check is required to sign up or continue working on our platform.
- Some states and/or facilities may require additional documents according to state law or facility policy.
How to upload your documents
You can quickly and securely upload your documents through the Clipboard app by following the steps below. Once all of your documents have been uploaded and approved, you’ll be eligible to start booking shifts!
- Go to My Account and select Documents
- Scroll down to locate your desired category (Expired, Required by all facilities, etc.).
- Select a document type and follow the instructions to upload your document.*
* Some documents will allow you to upload directly through the app, while others may require you to upload your documents through an external link. Accepted formats are jpg and pdf.
