INVALID DOCUMENTATION
What documentation do we consider invalid?
The following documents are considered invalid as we are unable to verify their authenticity:
- Call Logs
- Text messages
- Forwarded emails
- Photos of people
- Prescriptions
- Timesheets without a signature

WHY WE ASK FOR DOCUMENTATION
We need supporting documentation to process your Score dispute. Without it, your dispute may not be accepted, as we may not be able to verify your claim.

REASONS FOR DOCUMENT REJECTION
A non-exhaustive list of acceptable documentation for specific Score dispute categories is provided below. However, please note that we might reject your documentation if we are unable to confirm a document's authenticity. Some reasons why we might reject a document include the following:
- incompleteness (e.g., for a doctor's note: the provider's name, the provider's facility name, or your name is missing);
- illegibility (e.g., blurry, smudged, or unreadable document photos);
- inconsistency (e.g., the date on a document is incompatible with the date of a canceled shift);
- unverifiability (e.g., photos of car accidents or people or screenshots of text messages) or
- alteration (e.g., edited documents or images).

DOCUMENT UPLOAD GUIDE
Now that you have a document from the list above and double-checked that it meets all our requirements, let’s get ready to upload it. In order for us to process your dispute, documents need to be clear and legible so that our team can come to the best resolution.
Document types are broken into these categories:
Physical
- We require that all physical documents be clear and readable.
Screenshot
The screenshot must capture all the relevant information required.
Digital
Digital documents must be uploaded unaltered.
Photo (COVID Tests Only)
Photos of positive COVID tests must be taken from a “top-down” angle with the flash of the camera turned on in a well-lit area.
