As a healthcare professional, you reserve the right to add a facility to your DNR list if you no longer wish to work with them in the future, or remove them if you change your mind.
Before making a DNR request, please be sure to read our incident reporting process; if your complaint falls under one of the categories listed there, be sure to fill out our incident form and to make your DNR request there.
How to add a facility to your DNR list
If your reasons for wanting to add a facility to the DNR list do not fall under the applicable incident report categories, you may submit your DNR request by contacting our customer support team and providing the name of the facility and a detailed reason for why you wish to add them to the DNR list.
Removing a facility from your DNR list
If you should change your mind and want to give the facility a second chance, you will be able to do so after 90 days. Our Customer Support team will not be able to remove the DNR on your behalf before this 90-day window has passed.
